Translation is WAY better and it looks WAY better. But all the sexual stuff is removed and most of it is hacked back in with the +18 restoration patch. I'd recommend the official release w/ the restoration patch.
Just remember that you'll have the shittier soundtrack and all the CG's will be cropped. And you wont be able to see nipples through their armored suits anymore. That's about the only differences they don't fix.
