Medfag here. This is an exit wound, as evidenced by the "cracks" around the wound. This body has been dead for a long time prior to this photo being taken, if buried in a sealed container or area. The wound likely did not look like that with the "cracks" at the time of death, and it is the exposure of the skull that gives it such a dramatic look.

The other side likely does not have such dramatic "cracks", and I theorize that it does not have such a big hole. It is the pressure change inside the head and the direction of projectile that caused these "cracks". His eyes probably bulged out when he was shot, although whether or not his eyes remained bulged after the injury is unknown.

Conclusion: this body was shot in the head from the left side and the projectile traveled through the skull and exited on the right.
