Now that's what I call a proper increase in difficulty. Lunatic Sariel is a fucking monster, especially compared to her underwhelming performance on Hard mode. Her second phase (not shown) was a genuinely exhausting fight. Still, as with all final bosses, all you really need to beat her is a full bar of lives. I'll get my last 1cc soon and finally be ready to never touch this game again.

I'm fairly sure eirin doesn't generally give out medicine that can cause drastic changes in the body like that.
At best, we know she's given alice special sleeping pills, and thats about it. The rest of the medicine are actual remedies for the people in the human village and such.

Sariel's definitely harder, especially on Lunatic. Her patterns are more complex, and her second phase can make you lose a few lives. It's also worth mentioning that the stages leading up to her don't feed you lives like Hell-19 does, and YuugenMagan is a real dick kicker, even compared to Kikuri. Makai route in general is much harder than Hell.
