Three Medics and a Heavy working together. One Medic was healing the Heavy, a second Medic was healing the first Medic, and a third Medic was healing the second Medic. When their Ubercharges were ready they would take turns Ubering the Heavy so he'd be invulnerable for 24 seconds instead of 8.

You already recognized your key problem which is that spies attack when you're shooting at someone else. You need to abuse this problem to lure spies out. Do mock-attacks against imaginary enemies every now and then and quickly turn around to see if the spy's there to try and backstab you. Whenever not in combat, try to constantly look around and spycheck with your minigun. Be sure to spycheck good C&D hiding spots such as on top of rocks, tires and crates. When attacking, ALWAYS spycheck shortly after the gates open. Turn up your sound.
