That's exactly why games, that are fully voiced, don't let you go off the rails like that. Todd thought it was fine to just give everyone a voice but didn't consider what else needed to change. Same reason you got more or less the same control over the world as in FNV or FO3 but you have your own faction to command with which you can't actually do anything or make your mark on the world in a way that is clearly reflected by its inhabitants, not even by major story relevant NPCs.

I have hundreds of hours in 3 and NV but I can't get into 4.

I've played it on and off since launch but it feels like the power creep is so ridiculously slow, I don't like the power armor, I honestly don't like the voice acting, my dialogue display mod doesn't work, the dialogue is mostly awful and bland, ridiculous amount of trigger events. The whole game just feels like it's bogged down to me but simplified at the same time. How do people enjoy this game, is there any hope to?

SOTS just adds a bunch of new locations and dungeons in the Glowing Sea. Its eventual goal is Hades, a large underground complex of just about everything that was buried by the nuclear detonation.

No, FO1 and 2 didn't allow you to go off the rails like that. You got to make certain choices(becoming a porn star, kill children etc.) and it made a difference but your appearance didn't, except for the raider gear and leather jacket for certain quests.

so far as I can tell the skyrim one works with FO4, had some annoying sex mod shit baked into one of my save giving me a popup and running it through it worked

Its this right here, man. She doesn't like it for god knows why. Its just a mod installer that outclasses NMM and mod organizer. It even has loot built in for fallout 4. It also has its own mod depository.
I think she was even going on about how its stealing mods right off your hard drive at first but thats just it backing up your mod install in the cloud, if you have that feature checked.
It even allows you to share your mods with other people without having to upload them to a file hosting site and sending them a key.
The only real downside is its always online. At least its free though and doesn't slow your downloads down to 1mbps unless you goy up.
