In χ[chi], Ultimate Cards were created by fusing a High Purity SR+ (equivalent of a max Guilt 6*, I guess) with the various gems. Rainbow Gems were the base, and then you'd use the PSM gems on top of that, or the Beautiful Gem for Support Cards.
Ultimate Cards are ostensibly the same as HP SR+ cards, but they unlock a fourth Skill slot, and have a new strengthening mechanic.
Once Ultimized, your cards could now be offered SR+ and HP SR+ cards, which would strengthen the stats of the UC by something like 5% of the offered card's stats. Once you've offered five cards, you have the option to Rebirth your UC, which brings it back to the default stats, plus a percentage of the additional stats which had roll tiers like when applying EXP medals in Uχ. You can Rebirth three times if I remember right, and there were special Ultimate Moogles which had max stats which applied the max possible strengthening to a UC.
Guilt worked completely differently in χ[chi], so the card's base stats were significantly more meaningful and Ultimate Cards essentially created a power gap the size of 5* Medals and max Guilt 8 Medals between HP cards and Ultimate Cards.

You already know that's gonna be an official thing. Kingdom Hearts is a conglomerate of hearts, it's the heart of all worlds and all that lives. Sora is always connecting his heart to others, and so his 'power of friendship' is a literal thing. If Sora is connecting his heart to others and making these grand bonds between hearts, then "Kingdom Hearts is the friends we made along the way" is pretty much expanding upon that to it's logical conclusion.
And that's why Sora is, or will be, the living embodiment of Kingdom Hearts.

That's where Sora stepped in again (because they already had a connection) and Ven's full heart checked into Sora's Heart Hotel. At some point between BBS and DDD, Ven's heart was healed completely, and he's all ready to wake up. But to do so, they have to go get Aqua, who has to go to Castle Oblivion. Once they get to CO, they can go to the Chamber of Waking and go awaken Ven's sleeping heart. >Vanitas looked like Sora because his face re-shaped due to Sora touching Ven's heart? Basically, yeah.
