My actual understanding is that that most geishas did not, and do not, fuck their clientele most of the time. Geishas were initially not directly connected with sexual services. They were employed as entertainment for the guests of oiran, actual Japanese courtesans. That's why geishas were required to dress more modestly not to overshadow flashy, opulent oiran. However, oiran were mostly focused on just providing sexual services, while geishas were more educated and refined, and in 19th century started getting more popular, as they also provided more sophisticated entertainment.
