Use Keepass 2, change security settings so you're inputting the master password in virtual desktop and therefore immune to keyloggers.

Use emacs to edit a plain text file with the file extension ".gpg", and put your passwords there. Emacs will encrypt and decrypt the file for you when you need it using PGP encryption.
Works on Linux and Mac (with a little fiddling with homebrew).
