The problem is not so much with Bitcoin or crypto per se. The chance of creating duplicate private keys is vanishingly small, so small that the risk is acceptable, considering all the benefits of crypto.

What increases the chances of creating duplicate private keys is using insufficiently random random number generators. This is more likely what happened; the exchanges are using some crappy algorithms to create private keys, basically undermining the security potentials of the protocol.

Don't blame the tech, blame the lazy coders at the exchanges / wallet app writers.
