Lots of anarchists have realized this. It's a pretty common defense of the ideology.

Probably not all that different. It would probably be more decentralized, but Stirner was pretty clear that even the union of egoists would require forgoing freedom, and he's pretty clear that things like altruism and cooperation are essential to pursuit of self-interest.

It's worth noting that while he considered the state to lack intrinsic legitimacy, he never actually used the term anarchist, he was just influential on anarchist thinkers.
