Yeah he probably had hypertrophic cardiomyopathy (HOCM), this is a genetic thing, not due to exercise, but it causes sudden fainting and death when people with it exercise
Or exercise-induced cardiac hypertrophy. It's rare outside of serious cardio freaks and guys on anabolics, but it isn't actually impossible.

Exercise induced cardiac hypertrophy is physiological and isn't arrhythmogenic or dangerous. It also isn't rare.
All roids arent for size enhancing though, he had stamina and other qualities improved through roids
