Regardless, here's a few ways.

1. Your character is cursed to change genders every full moon.

2. Your character is a shapeshifter/changeling

3. Your character is a mage that excels in alteration spells.

Gameplay-wise, you could use the excuse of needing to charm or intimidate NPCs, I suppose.

While this is ostensibly possible depending on your fetish and how you play your character, unless your fetish is getting shit past the DM's radar you'll be much better off if you can find an ERP campaign that caters to your specific kinks. Try and see if any of your fellow internet degenerates are interested in one. If none are interested in DMing, then it will likely be up to you to do it since you're the one who brought it up in the first place.

If you don't have any internet friends who share your tastes or you're too lazy to DM then that's really just a whole other problem I'm not interested in solving.

Well, to clarify, I don't personally like inserting my kinks into my games. But if I were so inclined, my character could have a butt plug in without anyone else knowing* as I have done in the past. There is certainly an audience for long-term play rather than one-shots; and for plot-driven play rather than straight gonzo. Just gotta dig a lil deeper.
