Depends, MkIII helmets work just fine. I used a BaC terminator helmet on one of my contemptors, I've also seen a NL one with the skull champion helmet. What helmet did you have in mind?

Nothing in particular. I only have mkIV (BaC) and mkVII from 40k tactical squad available, I just couldn't stand if the two heads were the same. Oh, I also have Khorne Berzerkers, but the bunny ears would go off. Still, they feel a bit too small even if it's just a sensor array and not an actual head on the dread.

Personal preference more than anything. MKIV heads don't quite suit the IVth legion. Don't dislike them either, just don't have the right feel to them for the grunts.

Yes, 20-30cm distant multiple short bursts. Mostly it's okay but some gray spots are left with chaos black.

Well the plot holes wouldn't have existed had that been true from the start. Doesn't even need to be outright stated, could simply be implied.

Regardless the HH series as a whole is crap it humanizes characters that shouldn't be humanized and removes the mystery and obscurity to what was once an unknown era. At least FW portrays their fluff as though it is from historical documents that may or may not be accurate especially after ten thousand years of editing.
