Immortals Handbook by Upper_Krust. He had plans to make a whole set of Epic-level and God-level supplements for 3.5e and 4e, but he never ended up releasing anything besides the 3/4 finished bestiary and player's handbook. It's a shame, some of his weirder ideas, like the Umbrals were pretty interesting.
