Depends on which otherworld we're talking about.
Servant races (angels, demons etc) from the divine realms? No biggie, they're pretty much just god-created humans with extra bits anyway.
Netherworld demons? They're just netherworld's human-equivalent. Things might get a bit messy but will probably work out eventually. Cultural issues, incompatible magic and such.
People from outer fragments or deep nether worlds? Again basically humans, might even have common ancestors.
Deep nether denizens that do not hail from a world, but the nether itself? Your world is fucked. These guys are defined by them devouring the very concept of life itself, therefore their 'love' probably just means you and the world you're in look tasty.
Things from beyond the nether seas? Just having one pay attention to you would be bad enough. Love would probably involve your entire world getting reshaped into something that thing mistakenly believes would better suit you. In short, everyone's fucked.

Spirits technically have the same kind of souls as mortals, so while they're often insane from a mortal perspective or at least have weird priorities, they usually understand basic stuff like broadly what emotions are supposed to look like and why humans don't want to die, so things only rarely go totally horrifically.

That's kind of how we defeated the world-threat cthulu type entity. Devotees began a ritual which allowed it to coalesce and couldn't be stopped once started. One of the party without telling us though, changed the nature of the ritual a ways through into a wedding, to marry himself to it since the process was inevitable. The entity vaguely had to conform to the demands of the ritual, so became his happy and loving bride in a sense but certainly not appearance.

So he basically sacrificed himself because after that his "bride" drew him away to her realm to live happily ever after by her perception, and lavish him with affection. A pretty ghastly fate for him and it only served to kick the can down the road in terms of the world ending, because with him the entity started spawning children, and being half-material planar would be able to emerge without assistance into the world. The assumption was that some unknown amount of time, maybe centuries or millenia, eventually the domain of the entity would fill to the brim with children and burst at the seams, letting an army of nightmare monsters into the world.

She'll save state you. She'll disassemble and hack and replay the continuum of your existence because she wants to know what each instance of you would do in any situation. Because you appealed to something that could mine you for all your content and values your ecstasy and despair the same.

It vastly varies depending on the couple in question.
Most resistance usually comes from people living in the woodlands, far away from the magically radioactive desert, the ground zero of which teems with formerly human creatures that would make HP Lovecraft swim across the ocean to keep away from them.
Overall, the few eldritch creatures that do exist hold two forms, one for combat and one for interaction.
The combat form usually echoes their true form, while the interaction form has limited shapeshifting and usually looks like an otherworldly or slightly uncanny version of their closest race.
An eldritch in a dwarven mountainhome might have large, glowing yellow eyes and seaweed-like braided beard, for example, and one based around Junkers would look like Anubis through a negative filter. All of this is to make interaction less of a chore for the eldritch and less of a night terror for the normals.
Of course, most of that goes out the window with Shiners and the Twisted, afforementioned radiation dwellers who never look like each other even when ostensibly identical twins, and usually around these, the eldritch use their true forms because the Shiners and the Twisted have seen worse.
As for alien affection, most are aware enough that their love is not local love, and mostly just monitor other couples to get a feel for what is acceptable.
Those that don't usually end up trying to be helpful and going full Junji Ito in the process.

As much as this is a stupid fetish thread, this actually brings to mind something going on in a current campaign, where a player has waifu'd what amounts to an eldritch horror.

The creature he's formed a relationship with is a Doppleganger. Now, in this setting, Doppleganger are fey like creatures who lack that essential spark of life and individuality to be creative. They're fundamentally lacking that "thing" that lets them create or be original. Thus, Dopplegangers can only copy what they observe, and as a result they tend to be OBSESSED with humans, who are completely unique. This obsession can range from psychopathic hatred to yandere love.

Physically, they are in complete and constant control of their bodies down to the cellular level. They are essentially, 'It', though without the power of infection or division. This means they also don't experience truly reflexive or uncontrolled responses, like emotions. Instead, a Doppleganger will observe a stimulus, realize that a certain reaction is the proper response, and react accordingly (based on what they know). Thus, a very old and well learned doppleganger can pass effortlessly as a human, whereas a young or unlearned one will seem awkward and autistic. This doesn't mean their feelings aren't 'real', they're composed of the same chemical response as a human, but they're entirely intentional. Ex. Sad thing happens, a human experiences sad thing, becomes sad. Sad thing happens, doppleganger experiences sad thing, realizes "I should be sad", becomes sad. It's a subtle but somewhat horrifying distinction

The particular Doppelganger in question is obsessed with humanity as a fan, she/it loves them and is fascinated by their ability to create. Moreover she/it is fascinated by the players character. The character themselves is actually something of a milquetoast individual. It's this mundanity that fascinates her/it though, as she see's the PC as all that is quintessentially human - both banal and yet utterly unique.

Now, the players aren't stupid - they've taken precautions. This particular Doppleganger is their ally, and is moreover bound by powerful magical geas which prevents them from harming or allowing to be harmed humans (essentially enforcing Asimovs laws on it). The player in question who waifu'd it is kind of a sad neck beard and I didn't want to crush his hopes and dreams, so when he indicated an interest in her/it, she/it returned the sentiment enthusiastically. *VERY* enthusiastically. Enthusiasm that would be terrifying if this player had the sense to be terrified (which he does not, to my consternation)

To date, the Doppleganger has:

I keep trying to think of more and more fucked up shit to do without actually hurting anyone, or without making the guy feel like i'm victimizing him OOCly (he's a bit sensitive). However, he is stubbornly unaffected by all of this shit, and is just glad to have a waifu. It's as sweet as it is infuriating.

It's generally a very bad thing to be the focus of such an entities attentions, whether good or bad. However, some are (slightly) less cruel than others, and they may show their affections by granting a boon of some kind.

Well, considering only the Doppelganger, it's only linked to those with (a sometimes supposed) a one-sided love. That still make a lot of people outside that category.

MGE, to my possibly faulty recollection, has it that the "Spirit Energy" generate via doing the nasty with your monster waifu get's bounced on to her, and bounced back stronger, creating what is essentially a Perpetual motion engine of fucking. If you have a monster waifu, you won't die of old age. (This may be wrong, feel free to insult me) And a solid 90% of the monsters are inherently destructive. The ones I can remember that need to fucking die and the Nacth-somethinig or other (The Spider sperm sac world ending thing) and anything remotely similar to Matangoes, which are essentially Waifu'ed Human Cordyceps fungus.which is just as fucking horrible as it sounds.
Also, am I correct in remembering that the Encyclopedia entries are written with a darker tone to skirt pass the Order? cause I remember that from somewhere, then again, anything involving MGE get's handcannon'ed the fuck out of.

And since monstergirls tend to have a lot of sex, they have a lot of offsprings, so their pop increases greatly, furthermore reducing the "pure" human couplings and the amount of males available for the next generation.

Funnily, this was less prevalent in early entries, that focused a bit more on "this is a rpg monster, only it steals men to perpetuate the species" instead of "this creature's only goal is to aggressively hunt a man and fuck him for all eternity". I guess the author has grown comfortable with his fetishes over time.

In the long term I don't see that being the case. Females are the limiting factor in population growth and there's no reason why the men who stick with humans couldn't have multiple partners.
