If you use one regularly, just colour in some of the spots to make the number obvious.

3 or 4 = 2
5 or 6 = 3
This. The warhammer way. It has never come up as an issue or with any confusion in any games that I have played. It's gotten to a point when I know im rolling a d3, when a 2, 3, 4, 5, or 6 come up, i don't see those numbers. I see 1, 2, 2, 3, and 3 respectively.

wut? Just use n/2 rounded up. 1/2 = 0.5 rounded up to 1, 2/2 = 1, etc.
d2 gives an average result of 1.5
+1 makes it average 2.5
a d3 should be average 2
as people have said, a d6 is the best solution
if you wanted to use a coin, use 2d2, which will output a total of 2, 3, or 4. Then subtract 1 for outcomes 1, 2, or 3.

Compare to a d3:
1: 1/3 chance
2: 1/3 chance
3: 1/3 chance
These two things are not the same.
Roll d6, divide by 2, round up. How is this even in question?
Roll a d30 and divide by 10
Roll a d9 and dive by 3

Roll a d51 and divide by 17 is probably the best method.
