The brain has no pain receptors directly. You'd fill them breaking open your skull which I'm sure is painful, but the brain itself wouldn't feel anything.

There was an entry either in lords of madness or one of the underdark books talking of how a Mind Flayer had put a magic item on his victim so that they would have two way empathy.
And how the mind flayer was enjoying the wonderful flavor of the fear that built up as the victim felt the dulled pain of the tentacle secretions melting away at the scalp and burning into the skull, followed by the horrible realization as the victim realized just how delicious his brain is before death.

It hurts but it's quick. Once it's grabbed you with all four tentacles it takes less than 6 seconds for it to pierce your skull with its beak and suck your brain out.

From how it's described, illithid tentacles secrete a powerful acid that dissolves flesh and bone but not fatty tissue (like the brain), and they have sharp teeth to prise apart bone and hold the victim in place. It takes 6 seconds (10 if you use AD&D rules). 1 round. then you're dead.
