The only person who's dead is Konrad. What would be the point of literally four different endings to choose from, with only one of them having Walker be dead?

What? You hallucinate for most of the game, and Konrad is dead the whole time, but you only die if you kill yourself at the end. The three endings are:
1. You kill yourself
2. You call for extraction, and kill them all when they arrive
3. You call for extraction, and go home.

Oh, or you can get killed by the guys extracting you after you attack them.

It wasn't about having a choice, it was showing that war is hell and not just shooting bad guys, saying OORAH, and chesy-bumping your bros like most shooters portray it as.

It's absolutely not.
It was just a "it might be" by the author on twitter iirc, but the game doesn't make it explicit at all. There is just the déjà vu thing which is not much.

The lead writer personally believes he's dead and the game is him in purgatory.

But that's just, like, his opinion, dude.
