I do want some kind of deck that does what you call for though, but that would be like a deck-specific alternate wincon.
Orgasm first is a terrible idea in part for exactly what you describe. If you are making a sex game you want it to be sexy, not encourage both parties to make things as unenjoyable for the other as possible while furiously masturbating themselves.
The two real options are
A) The goal is to make the OTHER person orgasm first, or
B) The goal is to make the other person willingly sexually submit to you (which is basically the same as A but more thematically long-term and psychological rather than the game being just one long fuck)
While this thread is mostly bullshit, I actually really like the idea they're using in Consentacle, whenever that actually ships.
The basic idea is that two players have to try and cooperate, each player providing tokens of one type that, when they fit together, form points you score together. There are restrictions on how you can communicate, so you need to try and hint subtly what sort of card you're going to play, going for the best matches possible to produce the most mutual satisfaction.
In terms of taking the idea of physical and emotional intimacy and making actual mechanics around it, it's the best version I've heard of up till now.

So effectively Positions are a kind of combination of field spells and enchantments? Are they something each player keeps in their deck, or drawn from a communal pile or secondary deck? Additionally, Heroes are definitely going to be the big draw since they'll probably define how deck archetypes work. Maybe separate them into different "themes" of hero? Like you could have the "BDSM Faction", but then within that have the Dominatrix, the Gimp, the Shibari Master, etc, each with their own particular style of play and deck they're generally suited for out of the box.
I quite like it though user, I'd be definitely interested to see where this thing goes.
I got the idea for Positions from the Call of Cthulhu where there were communal missions that each player could assign units to in order to fight for to win the mission. I plan on them being in the players' decks though. I definitely plan on having heroes with themes like you outlined. This thread is to get more basic gameplay ideas.

They're simultaneously attack icons and defense icons. Battle is done through moves which invoke a fighter to attack and another to defend which specific icons.

There is 1 hero per deck and many minions within the deck. I was playing around with a roster of heroes, which I guess is pokemon's style. 1 Master hero as Lifepoints/Deckmaster and sub heros with stunted stats in deck make the most sense at the moment.
I'm sure this will get so much actual play between two actual real people.

Maybe less sub-heroes and more subordinates? Less of characters, more of crew I think. Like the Slut class might have a Crew card named "Boy Toy" that does something, probably based on attention, while a Dominatrix would have Slaves or something like that. Lots of variety, but the idea is that they're not quite important enough to be named sub-heroes, unless they're the equivalent of Legendary Creatures and even then I'm not sure if those should stay crew and instead be their own Heroes.

There are several types of cards:
- Arcana, which remove clothing

These are your bachanallian party animals, and these are the ones who get a lot of group sex focused abilities and positions. Including possible archetypes like the Slut, the Player, the Party Animal, or the Porn Star they run hard aggro, looking to do as as much damage as they can and tie up as many other characters as they can along the way.
Future expansions could involve expanding on these categories with new heroes and cards, as well as adding on additional groups if we can figure out good ideas for them.

I'd add a "guess my fetish" and/or "fulfil my fetish" sub-game, with card having either a better effect when targeting someone with a specific fetish (or fetish type), or alternatively having no effect unless the target has a specific fetish.
In a multiplayer game, debuffs like "STD markers" could be introduced, which would on one hand hamper you (you can't play certain cards if you're so debuffed, maybe suffer other penalties), but on the other hand make it so certain cards played against you mean the "attackers" get the STD debuff too.
