This trope is literally the only good thing about SM Stirling's "Dies the Fire" series, aside from the heraldry jokes. The main bad guy is a SCAdian fighter, who goes full Dark Lord and adapts to the suddenly medieval setting really effectively, so he ends up as the Badass Warrior. His wife goes full Scheming Sociopathic Political Bitch, and is arguably more feared than her husband.

The King is a human hero chosen from the common people or petite nobility. The Eternal Queen is an immortal (dragon, demon, angel, fey, etc.) who uses him as a breeding partner to continually infuse the nobility with her mystical heritage via political marriages.

Meh, the nobles are the ones who push the idea she has to be of good breeding. In reality any fertile female would do. The King of Ages returns to his slumber after the Kingdom is stabilized and the maiden/sacrifice is impregnated. The child born of their union is typically powerful in their own right, defends the Kingdom once they come of age, and typically marries into one of the noble families.
The Nobles live in fear that the secret will get out. If the people realize they are superfluous to the process of awakening the King of Ages, then they might decide they are superfluous to running the Kingdom as well. The King's Shrine is guarded by an order of Knights sworn to kill any unworthy woman who approaches.
Two hundred years ago an ambitious Sorceress (pic related) got inside the shrine and woke the King. Sensing that she was the threat to the Kingdom at that time the King gave her two options: impregnation followed by immediate execution, or impregnation followed by exile. She chose exile, and her bloodline now rules an allied Kingdom.
