My campaign is centered entirely on that concept. Since during the middle ages there was an adventure craze, whenever the pcs enter a dungeon, I secretly roll a d100. Depending on the results (the table is adjusted depending on the dungeon, so a dungeon close to a city is almost guaranteed to be empty), the pcs may find absolutely nothing, or be able to loot the corpses of their enemies or previous adventurers. It adds some risk in choosing where to go next, prevents the party from becoming rich, and further enforces that being an adventurer sucks, so I like it

So why in the world would ANYONE become an adventurer in such a setting? The entire point of adventuring is that it's high risk, high reward. You have like 90% chance of getting killed(PCs being exceptional have better chances of survival, obviously), but if you survive, you're going to be filthy rich.

Most people don't want to, there are barely any adventurers. It's basically what you end up doing when you're really desperate for money or for a sense of purpose, or are just a criminal who can't stay in a city. The few adventurers that do exist are usually pretty fucked up, or where around during the adventuring craze and don't want to stop being adventurers.
That said, there's still a tiny chance that an adventurer can find some really valuable loot if they're lucky enough, and adventuring isn't just dungeon raiding.
