Equal parts butter and cream in a saucepan. Simmer until a lot of the moisture has been removed and it starts to thicken a little.

Add a generous amount of parmigiano-reggiano cheese. Season with fresh ground black pepper (I like quite a lot of it), and a little nutmeg. Salt isn't needed because the cheese is salty enough.

I sometimes add chunks of grilled Italian sausage if I want to make it extra-fancy but that's not needed.

Other spices? No. If the cheese and the butter are good quality then you don't need much else.

If I'm feeling fancy I might use a little parsley or chives finely chopped as a garnish. I don't add garlic. I love garlic, but IMHO it doesn't go well with Alfredo.

Does this work with penne pasta as well?

i kinda like using sour cream instead of plain heavy cream, it has more flavor.

just heat it up and simmer it for about 10 minutes to thicken it a little, add parmesan (or romano, i prefer romano) until it tastes good to you. simmer until thickened to your liking.

add black pepper and parsley if you want.

you reduce the cream a bit, and dry pasta triples in weight after being cooked. so no, it's not 2:1.
