I need to start studying in the library or something because I have no self-control. I normally do fine, but recently my motivation had plummeted and so it's way too easy to spend all day not getting anything done.

I'm actually falling behind in my classes and just took a couple midterms that I never even studied for. I'm not in a good spot.

It is the same for me, both the porn and Veeky Forums, and I got through most of Med-school (4th year now) and 2 degrees (B.Sc. and M.Sc.). The trick for me was to compartmentalize.
1. I carefully compartmentalize my schedule (to the level of what I want to accomplish every hour) in practical goals
2. I plan time for some fun in evening
3. I use buffer time
4. I block distractions

For instance, during the exam period I generally need to read about 120-130 pages/day to be ready for serious exams (like pathology- 650 pages in 5 days, or microbiology- 550 pages in 4 days). I read at a maximum pace of 15 pages /hour probably (depending on subject), but I plan for 10/hour, and define the study time from 09:00 AM to 00:00 AM (15 hours rather than 12-13). This both allows buffer for distractions at the single hour level (Veeky Forums, staring) and at the daily level (fapping, pooping). I keep reupdating my schedule according to progress. I do all the studying in my apartment, but for especially difficult exams I usually spend some time at my parents' place. I made my room distraction free: I have a place for my phone and TV power cable outside, and I periodically check the phone every hour. I use a chrome extension to block all porn and words related to it, I block sites like facebook and Veeky Forums that prove distractions, and I sometimes have my girlfriend watch over me to make sure I study.
I then spend a focused 1-2 hours doing something fun, before getting read for bed.

During the semester it's obviously not that crazy, but I follow the same principals - defining a clear time for studying, planning it meticulously, making it as distraction free as possible, and planning some time for fun.

It's nice that you can actually control this.

What I do is I go through cycles of

1. Have my desktop put away in the attic under a pile of crap, so I can't access it.
Without a computer I study, read stuff and do all things I should be doing, until

2. I get too lonely/bored and set up my PC again.
Then I waste an ever-increasing fraction of my day on the internet until I realise I'm late on a couple lab reports and out of laundry for the past week.
Then I put away the computer again and go back to 1.

I have a really fucking bad internet addiction issue. It's the result of relative poverty (thus inability to socialize properly) and it's not easy to "just" break out of.

I see, that sounds harsh. Obviously I don't know how it is like for you, but personally when I tried to go completely distraction free back in my first year and packing my laptop, I couldn't last long. I think the key, at least for me, is the compartmentalizing. If I don't have that 1-2 hours at the evening to surf the internet or go out with friends, I won't be able to last. I also check the news all the time, if I completely block my access to news I give up after a couple of hours of studying, but by accepting that I'm probably going to spend maybe 5% of my study and work time on news-distractions, I can plan better.

Don't know if it will work for you, since it sounds like you really need the internet, but I wish you the best of luck anyways.

I throw my internet cable under the table when I want to get some reading done, then any impulse to check some website gets thwarted quickly because I can't bother crabbing under the table to plug it in

same, but resulting from a combination of depression/study anxiety.

maybe try using the pomodoro method, it helped me a lot. set a timer (or use an app, these also let you track how much time you spend studying) for however long you want to study for. start with as little as 15 minutes if you have to - i usually do 25 but you can up it if you prefer to work for a longer period, my friend does 45. you work until the timer goes off, and when it does, you take a break for a set amount of time - walk, eat, piss, goof off on the internet. then after that's up, you start studying again. it's surprisingly helpful

but really i just wish i had a friend with the same problems so we could be frank with each other and bitch each other out for getting distracted
