@1, it's no more or less efficient as long as you're not missing optimal shift points because you hesitantly avoid steering with one hand when the conditions call for it. It would make it harder to manhandle the steering and naturally incentivize you to avoid getting the car too far out of control. It will also train against the first thing I mentioned, in that you should know how to handle the steering just as well while upshifting or downshifting into corners.

@2, if you aren't exerting any force on the shifter I don't believe it'd be bad for the car, but I'm not mechanically knowledgeable enough to say if leaving the weight of your hand on the shifter at all is detrimental to the transmission. As for general safety, two hands can handle extreme situations more effectively 100%, but shifting or lack thereof can also cause losses of control or aide recovery, so it's not a good thing to ignore if you have the ability to shift.
