Memories are stored inside your brain, so yes, if your brain was magically transplanted into another person's body, you'd be "yourself" in another body. I think that personality comes partly from conditioning and environment, i.e. the brain

I think that would be horrifying, even if you moved to a younger, stronger, more attractive one. There's something about spending your whole life in your own body that makes you not want to trade it.
I'm not saying that it's plausible to transfer the brain from body to body, but for argument's sake, the brain IS where the personality is held, so if you had the same brain, logic holds that you would keep the same personality.

What if we removed the brain but still kept it alive? Would the person inside be fully aware, essentially trapped in a prison with no sight, hearing, feeling, smell, or any of the sensations that the body brings? I imagine it would be like floating in a black void with no way out.

It is theoretically possible to do this, but we don't even know all of the things that our body does to keep our brain operating correctly/healthy/repaired. Our brain is a super computer, and our body is the case and monitor. Imagine having the most beast computer components ever created, but not even being close to having a case and peripherals that could adequately show what it was processing or even keep it maintained.
