You only have to count the bar if the serial numbers on the plate and bar dont match.

a regular olympic bar was designed to fill holes in 4 45lb plates, because 2pl8 is pretty much the maximum weight most people lift
a 405 squat is actually about 360 unless you lift using two bars

No, it means that if the serial numbers match then the plates and bar are properly calibrated so you can calculate the weight propperly (ie: "1 plate = 135 Lbs").
