For example a bench press for the max at a exhibition allows you to arch your back and typically would not follow the 1 second postive 2 second negative rule. Good form would add difficulty.
Cheat reps are really good at the end of a workout or at the end of a set. I usually go low weight high volume when I cannot do any more reps or throw cheat reps if it's standing exercise. Another good trick to break a plateu is superset the concentric motion like hammer curl to tricep extension and exaggerate the negatives.

I have a whole day for shoulders, see? Forearms get a bit of a workout with biceps/back but I'm a climber and I weigh 85kg so my forearms are already disproportionately swole. Do ab wheels to compliment heavy lifts for core (and leg ups when I do pullups).

Because they don't do Crossfit (or at least not the way its generally sold). They train in much the same way that athletes in general do because they typically treat Crossfit as a sport - large amounts of GPP and skill training, not rocking up to do a random workout and then buggering off for the day.
