Basically what I'm trying to ask is if this sort of situation would be possible to avoid without surgery after a massive weight loss. I mean, the skin doesn't return to normal once it has been stretched too far, right?

Skin has a fair amount of elasticity so for it to be that bad you'd need to get pretty fucking fat
