I prefer my settings be consistent. If it is serious grimdark keep it serious if it is tongue and cheek grimdark keep it tongue in cheek. Don't have a serious grimdark setting and also have Killbane the Skull Lord who only drinks from the skulls of his enemies and keeps demons (blood thirster style not succubi) chained up in his dungeon to use as sex slaves. Likewise don't have a grimdark setting with a race of Tom Bombadils.

That being said my setting isn't really grimdark, I tried to make it a relatively culturally accurate early medieval style setting. Sure slavery exists and climbing the social ladder is hard if not impossible for some with endemic warfare being common but your odds of dying violently are still relatively low and humanity shows no signs of being in danger as a species.
