It means, falsely, that there is nothing that is identical to itself, which is a roundabout way of saying that nothing exists (since it is denying a universal property that is satisfied by just about every conceivable object you can think of.)
To say that the binary relation = is irreflexive would require you a Second-Order quantification, which is a different kind of Logic from the one is used here.
