There's plenty of video of divers being caught against grates/inlet pipes by much less "exciting" ΔP.

30 psi (~2 atm) of suction on a 4" sewer pipe is ~1500 lbs of force. Maybe not enough to turn you into LCL, but enough to break bones and leave you trapped, underwater, completely helpless.

Let's say there's a grate in front of the sewer pipe, and you only block off half the flow. That's still 750lbs of force; you'll probably survive if you can get help, but there's no way you're removing yourself from the grate solo.
