I almost always specify by the number on the menu, like at wendy's I always get a small number 6 with Dr Pepper.

I'd guess maybe half of people use the number when they order a combo. Many people will even ask like "Do you guys have a Whopper Jr. meal?" despite it being up there as a number 7.
